This online form can be used to pay your membership dues if you are applying for Individual membership. If you are applying for institution, corporate or student membership, you must submit an application via fax using the provided PDF forms or call AAMI at 800.332.2264 Ext. 1214.

2. Sign-up for AAMI Individual membership by using our easy online form.
To begin you must first have an existing AAMI account. If you already have an AAMI account you must first login.
If you do not have an AAMI account please create one. Then return to the AAMI online form and follow the steps.
3. If you wish to submit your membership application via fax or mail select the appropriate application form (pdf), complete with payment information, and fax to 703-525-1424 or mail to:
